Procedure for prompt issuance of physical handicapped certificate

Attend the District Civil Hospital/Medical College along with the following documents:

1) Two copies of Identity Card Size Photographs
2) Residential Proof ( Ration Card/Electric Bill/Domicile Certificate etc. )
3) Purpose of requirement of Certificate.
