Reproductive and Child Health Phase -II

Training activities under RCH II (Revised)For Medical Officer & Paramedical Staff various types of hands of training has been arranged in the State to improvise their skill and technical knowledge. Due to improvisa tion in various skills will get available to community at PHC & sub centre level.
The various key training are as below :-

1.   Skilled Attendance at Birth Training   
            Skilled attendance at Birth training will be imparted to -
  • ANM of PHC HQ as well as subcentres regular & on contract
  • LHV of PHCs Regular & on contract
  • Staff Nurses PHC on contract
  • Staff Nurses RH
Venue - PHC/RH/SDH/DH/Women Hosp. or Accredited Private or NGO Hosp. with more than 50 deliveries per month.
The duration of the training was 15 days. As per GOI instructions it has been increased to 21 days since April 2009

2.         Basic Emergency Obstetric Care Training
Basic Emergency Obstetric Care Training is  being  imparted to -
  • Medical Offices of 24 X 7 days PHCs ( for 2009-10 ELA will be 50% of the total no of PHCs )
  • Medical Officers of all RH & SDH

    Venue - DH/Women Hosp. or Accredited Private or NGO Hosp. with more than 50 deliveries per month.
    The duration of the training was 15 days. As per GOI instructions it has been increased to 21 days since April 2009.

    SAB/BEmOC Trg. Is conducted at 67 Trg. sites. Every month one batch is being conducted at District Hospital and Women Hospital alternatively of BEmOC followed by SBA

3.         Comprehensive Emergency Obstetric Care Training
CEmOC training is to be given to MBBS Medical officers having more than 5 years of service so that with the Emergency Obstetric care FRU/SDH will be made functional.
  • MOs from RH/SDH                                                                
Venue & duration – 6 weeks at Sevagram  Wardha /BJ Med College Pune /GMC Aurangabad & followed by 10 weeks at designated district hospitals. (BJ Med College Pune /GMC Aurangabad will start functioning from July 2009 onwards. Presently only MGIMS Sevagram is operational.
The duration of the training is 16 weeks.

4.         Life Saving Skills in Anesthesia (LSAS)As per GOI guidelines 18 weeks certificate course for M.B.B.S. Medical Officers who have completed 5 years services. It is being conducted at selected 9 medical colleges.
  • MOs from RH/SDH                                                                
A batch of 4 doctors will be sent to each Medical College.
Venue – 12 weeks training will be given at Medical Collage and 6 weeks at District Hospitals. The centers are: - KEM Hospital & G.S. Medical College Mumbai,  LTMG Sion Mumbai,  Dr. D.Y.Patil Medical College Nerul Navi mumbai, PDMC Amaravati, GMC Nanded, IGMC Nagpur, GMC Dhule, GMC Miraj, MGIMS, Sevagram, Vardha.

5.         CAC (MTP/MVA)

            IPAS, an International Organization has extended its support in providing Comprehensive Abortion Care. Manual Vacuum Aspiration (MVA), Medical Abortion has been included in the syllabus of this training.

6.         Integrated Management of Neonatal Childhood Illness 

Integrated Management of Neonatal Childhood Illness is the centerpiece of the Newborn and Child Health Strategy in RCH Phase -II. It includes the home and community based component (through ANM and AWWs) and the facility based out patient care component, Management of Sick Neonates and Children in the inpatient setting at PHC/ R.H./ FRUs. Health system strengthening and community participation components have been addressed effectively to ensure effective implementation.

IMNCI initiatives is being implemented in State as a major step towards reduction of Infant Morbidity and IMR in selected 9 districts in Phase I. These districts were Osmanabad, Latur, Chandrapur, Nanded, & Nandurbar,Thane, Nashik, Amravati, Gadchiroli. In Phase II Raigad, Beed, Ahemadnagar, Jalgaon, Dhule, Bhandara, Akola, Washim, Pune, Gondia, Yavatmal. Nagpur, Wardha. Districts. Dist Buldhana were included. During 2007-08 14 more districts were added. TOT was supported by UNICEF from 2006-07 onwards till today. TOT for SR. officers is being conducted at Kalavati Saran Children Hospital, Moulana Azad Medical College, New Delhi. At present IMNCI is being introduced in all the 33 districts. 11.       Routine Immunisations 

a)         Conducted for 2 days for all health workers i.e. ANMs/LHVs/ MPWs/ HAs including contractual ANMS/SNs. TOT is supported by UNICEF.
b)         Recently Trg. for MOs on RI has been initiated by GOI with support from UNICEF 7 WHO. During April 2009 we have completed 2 batches of TOT for HFWTC faculties & MO DTTs  (duration of trg. 3 days) at HFWTC Pune and PHI Nagpur MOs trg. will start as soon as trg. modules are made available.  
Venue – All DTTs /HFWTCs.
The duration of the training is of 3 days.

12.       IYCF  (Infant &Young Child Feeding )

Infant and young child nutrition training which mainly focuses on importance of early exclusive & extended breast feeding practices in lactating mothers. This training is supported by BPNI & Unicef. All 33 districts are covered in this training in 4 different phases. 1/3 districts have been covered in 3 phases. About 5 to 6 districts are covered in all 4 phases. The training consists of advocacy, training of Pediatricians, MOs, LHVs, ANMs, AWWs & lactating mothers. Last phase of the training is assessment of AWW in knowledge and practices of breast feeding.

13.    RTI / STI training of laboratory technician & Medical Officers, Staff Nurse, ANMs / LHVs
         Though this topic was covered under NACP (Family Health Awareness Campaigns ), under IPD Project and STI/RTI sub-project in Nasik district, the approach during the training was syndromic management There has been a considerable period after this training were orgsanised, hence it is necessary to train all MOs and paramedicals  under RTI/STI. Further this is an important component of Adolescent Health and as a preventive measure against HIV/AIDS. It is necessary to impart this training to all medical officers and paramedicals in the state. State has started RTI / STI training as per GOI norms during 2010-11.

laboratory technician Priority will be given to train all 24 x 7 PHC & Lab technicians from RH/SDH and newly appointed Lab technicians.  Training duration will be 6 days at district hospitals.

Medical Officers Priority will be given to train all MOs from all 24 x 7 PHC & FRUs .Duration of training will be 3 days at HFWTC. During the 2010-11 it is proposed to train selected 640 Staff nurses from District Hospital and also from FRUs will be trained for 2 days at Nursing schools.  During the year 2011-12 it is proposed to train 4960 ANMs / LHVs from 24 x 7  PHCs  and SCs.  Duration of training will be 2 days at DTT.

14.   IMEP Training Paramedicals staff (ANM / MPW / LHV/ Sweeper)
               

As universal safety measure and to protect stake holders, MOs & field staff working at PHCs and SCs need to be sensitized on Infection Management and Environment Protection with special focus on Hospital Waste Management. It is proposed to complete this training for all medical officers working at PHCs and Rural Hospitals.
It is proposed to train all paramedicals including ANM,LHV,MPW(M) HA(m) Supervisors etc. This number is 30000. The proposed training period is one day. The training will be conducted at DTT, Block level and RH level for 1 day.

15.       Navjaat Shishu  Surksha Karykram  (NSSK)
The  Government of India keeping this in mind has developed a program on " Basic Newborn Care and Resuscitation " which would address these causes in a large way .

With more and more deliveries occurring at health care institution as a result of the Janani Suraksha Yaojna, it becomes important that the large number of health professionals attending to births, are trained in the simple procedure of resuscitation and basic new born care details of these are.

  • Prevention of  Infection  / Hand wash
  • Thermal protection / Hypothermia Management
  • Feeding of normal and low birth weight babies

  • The NSSK training is a specialized training to address interventions of care at birth i.e. prevention of Hypothermia, Prevention of Infection early initiation of Breast feeding & Neonatal Resuscitation
  • Newborn care and resuscitation is  an important starting point for any neonatal program and is required to ensure the best possible start in life. This training is being introduced to arrest the high neonatal mortality in the States.
